Supporting the Whole Person
Tailored Care Management is a North Carolina initiative that focuses on supporting the whole person. Members who have mental health challenges and substance use disorders will be assigned a designated care manager who will help address all their needs (including mental/behavioral health, physical health and unmet health-related resources needs).
Program Overview
Medicaid recipients who are assigned Tailored Care Management services also have a choice of who provides the service. We have many years of experience and dedicated teams who support individuals with mental health, housing, physical health and resource related needs. For children and adolescents, we understand that supporting the individual also includes supporting the family. Our Tailored Care Management team will coordinate with you and your current providers to ensure your needs are being met and working towards an individual’s whole person care, health and stability.
