An option for staying in your home

Community Living Facilities and Supports is a residential service. It offers an option to Medicaid-eligible individuals with individual and/or developmental disabilities (IDD) who also meet Intermediate Care Facility (ICF) Level of Care but do not have an Innovations Waiver, to continue living in their community with the choice of residential options and meaningful day activities.

The Individual and Community Services team supports individuals with intellectual and/or developmental disabilities (IDD). We provide one-to-one services uniquely tailored to help each unique person acquire and maintain daily living skills.

Community Living Facilities and Supports (CLFS) promotes residential community inclusion with home and community-based principles that focus on individual’s personal desires to live in their own homes or apartments, living with family members, living with support staff or in residential settings.
