Transition Management Services

Transition management to help individuals live independently

Transition Management Services is a service provided to individuals participating in North Carolina Department of Health and Human Services’ Transition to Community Living Initiative (TCLI). TMS is rehabilitation services to support an individual’s ability to live independently in the community and maintain housing.

The Transitions to Community Living Initiative (TCLI) provides eligible adults with serious mental illnesses the opportunity to choose where they live, work and play in North Carolina. This initiative promotes recovery through providing long-term housing, community-based services, supported employment and community integration.

Transition Management Services (TMS) focus on increasing an individual’s ability to live as independently as possible in the community, while managing illness and reestablishing their valued community role. Easterseals UCP focuses on providing services that support the individual in the following domains: emotional, social, safety, housing, medical/health, educational, vocational and legal.

The Easterseals UCP team supports individuals participating in the Transition to Community Living Initiative (TCLI) and provide structured rehabilitate interventions that are focused on tenancy/housing, money management and entitlements, activities of daily living while promoting personal health, wellness and recovery and community integration.
