Family Navigator

Navigating family advocacy and decision making

Easterseals UCP’s Family Navigator service promotes self-determination, supports individuals in making life choices, provides advocacy and helps individuals with intellectual and/or developmental disabilities identify social determinants of health that need resources in order to become a part of their community.

The Individual and Community Services team supports individuals with intellectual and/or developmental disabilities (IDD). We provide one-to-one services uniquely tailored to help each unique person acquire and maintain resources for social determinants of health.

Family Navigation Services provide support to individuals and their planning teams to develop social networks and connections within their local communities. We also emphasize, promote and coordinate the use of general resources to address the individual’s needs in addition to any paid services.
