Easterseals UCP, is a non-profit service provider in North Carolina & Virginia. We are seeking a compassionate, caring, and dedicated Youth Care Coordinator (Wraparound Facilitator) to join our collaborative, multidisciplinary team designed to provide crisis interventions and support to children and adults.
What You’ll Do:
The Wraparound Facilitator leads team process in the delivery of the evidence based model, High Fidelity Wraparound (HFW). The Facilitator works with the other Wraparound team members and the youth and family team to identify the goals that youth and families wish to achieve. The facilitator facilitates plan development with the family, youth, their team of supports and the Wraparound team member to achieve those goals. Facilitators also drive collaboration and integration with other services and agencies (clinical services, Social Services, Juvenile Justice etc…) that families may be working with in conjunction with Wraparound. Wraparound is a community based service, providing our process model to families in their homes, in their communities and honoring of their schedules to receive our support. The position has a need for strong writing skills and ability to manage timely completion of administrative and documentation activities. If you have a heart for facilitating youth and families to identify their goals, develop their plan for success and assist them in making sustainable life changes, this position is for you! Counties that you will serve are Wayne, Sampson, Duplin, Lenoir, Greene, Wilson, and Edgecombe.

What We’re Looking For:
To join our team, you must be willing to provide care in a crisis group home and mobile setting, have a passion for helping others, have experience with MS Office suite and data entry, plus strong verbal and written communication skills. We also require the following:
· Masters Degree in Social Work, Psychology or related Human Services Field
· Provisional License or Licensed (LCSW, LMFT, LCS, LPA, Psychologist) preferred
