Tailored Care Manager

Easterseals UCP is a non-profit service provider in North Carolina & Virginia. We are seeking a compassionate, caring, and dedicated Care Manager to join our collaborative, multidisciplinary, Tailored Care Management team (TCM) designed to provide whole-person, integrated health care.

What You’ll Do 

The Tailored Care Manager is responsible for promoting whole person, integrated health care through evaluation, assessment, care planning, coordinating, tracking, and monitoring outcomes for individuals served.  The focus will be on closing gaps in care for physical and mental health, social determinants of health, and other non-health related needs. 

How You’ll Benefit  

You’ll earn a competitive salary based on your experience between $52,000 and 55,000 annually, plus enjoy a flexible, work from home and in the community position, Monday through Friday, full-time, 8am-5pm with flexibility to provide early evening coverage as needed for client availability, with local travel with-in Sampson county. We also offer benefits to those regularly working 30 hours or more per week.

What We’re Looking For

To join our team, you must be willing to provide services in various community locations as needed, rotate after hours support coverage, knowledgeable in Microsoft Office Suite, experience with Electronic Health Records software, have a passion for helping others, and able to link clients to community resources. We also require the following:

  • A Bachelor’s degree; Master’s preferred
  • Able to be credentialed as a Mental Health Qualified Professional
  • Minimum of 2 years of experience with the mental health and/or substance use disorder population
  • A valid driver’s license, current auto insurance and a good driving record
